- Identifying Information: your minor's name, physical address, date of birth, social security number, contact phone number.
- State issued ID card (such as a driver's license or permit) or a copy of their birth certificate.
- You MUST be the legal guardian of the minor.
- If you are not a current First Financial Bank customer, you will also have to bring the identifying information listed above.
